Added a FIRSTAID ELEVATE <aff> command.

This only works if you have the affliction known to you, or predicted it. It will elevate the affliction to the top of the priority list, even above priority 1 afflictions.

The elevated priority will be cleared when the affliction is cured, or attempted cured.
